Waimānalo Health Center patients may request a copy of their medical records. To do so, please follow the directions outlined below.

Please note: Waimānalo Health Center has partnered with HealthMark Group to manage medical record requests. HealthMark is a trusted partner that specializes in facilitating medical record release and keeping your personal health information safe.

 

Medical Records

Option 1: Submit a request online directly through HealthMark at https://requestmanager.healthmark-group.com

• Enter your email to start the process. You'll receive an email from HealthMark to log in. Click on the "submit request" button and follow the prompts. As soon as your records are available, you'll receive an email with a secure link to download your records.

Option 2: Submit an Authorization for Release of Health Information form to Waimānalo Health Center

• Download and complete form: General – Authorization for Release of Health Information
• Submit your completed form in-person, by fax, or by email.
  • In-person at Waimānalo Health Center
  • By fax 808-259-7447.
    • Please include a 10-digit phone number so we can contact you if we have questions.
  • By email to MedicalRecords@waimanalohealth.org.
